Become a Patient with Goodwin Home Health
If you are looking to use Goodwin Home Health services, you will need to submit a physician’s referral.
Please include the following documentation with all home health referrals:
- Patient demographic sheet with patient’s full name, address where they receive home care services, and insurance information
- Referring physician’s name and phone number
- Physician order for needed home health services
- Patient’s recent physician’s note, history and physical or hospital paperwork
